How to Add a Survey Title

The survey title feature provides a title area above the questions, like this:

This feature is optional and is 'off' by default.

Navigate to 'Customize' and add your desired title into the 'Survey Title' box:

Be sure to save your changes and the title will now appear above your survey questions.
